Black Hawk Down

The 1994 Black Hawk shootdown incident occurred on April 14, 1994, during Operation Provide Comfort (OPC) in northern Iraq. Two US Air Force F-15 fighter aircraft, under the control of an AWACS aircraft, misidentified two US Army UH-60 Black Hawk helicopters as Iraqi Mil Mi-24 helicopters and fired on them, killing 26 military and civilians on board. A subsequent USAF investigation blamed the pilots for misidentification, the AWACS crew for inaction, and inadequate integration of UH-60 operations into OPC air operations. Several USAF officers received administrative discipline, but only one was tried by court-martial and acquitted.
